Аннотация:Purpose To evaluate the relationship between the GCL thickness and duration of disease in patients with different visual pathway lesion.
Materials and Methods The GCL thickness was measured by optical coherence tomography (OCT) in 108 patients with a known time interval since visual pathway damage. Nine patients were diagnosed as optic neuritis (ON), 7 - posterior ischemic optic neuropathy (PION), 5- traumatic optic neuropathy (TON). Six patients was a result of traumatic lesions of the optic nerves and chiasma. Fifty two patients were examined for postoperative homonymous hemianopia after selective amygdalohippocampectomy for treatment of pharmacoresistant mesial temporal lobe epilepsy, 13 patients were diagnosed as occipital cerebral infarction, in 16 patients were reviewed for postoperative homonymous hemianopsia occipital cerebral tumor removal.
Results: Latency from onset of disease in patients with optic nerve damage ranged from 5 days to 20 years. The GCL thinning was demonstrated in all patients (16) with 14 days or more duration of disease. There were no difference in patients with ON, PION, TON.
Latency from trauma of the optic nerves and chiasma ranged from 10 days to 20 years. The GCL thinning was demonstrated in all patients (5) at a month or more following trauma.
24 patietnts with retrogeniculate visual pathway pathology showed GCL thinning ipsilateral to the side of damage. Patients with postoperative VFD after amygdalohippocampectomy were examined in 7 days – 3 years after surgery. Partial homonymous superior quadranopsia without VFD within 20° from center associated with normal thickness of the GCL. Patients with homonymous superior quadranopsia, complete or near complete homonymous hemianopia (11) showed the GCL thinning at 3 months – 3 years following surgery.
Duration of homonymous VFD ranged from 1 day to 14 years in patients with occipital cerebral infarction. The GCL thinning was found in 5 patient at 3 months or more following the stroke.Patients with postoperative homonymous hemianopia after occipital cerebral tumor removal were examined in 1 day -2 years after surgery. The GCC thinning was demonstrated at a year or more following surgery.
Conclusion Patients with lesions of the visual pathway show thinning of the GCL correlating with location of damage and duration of visual impairment.
