Аннотация:Introduction: White matter hyperintensities (WMH) arefrequently detected in migraine patients, however, theirsignificance remains uncertain. Objective: To evaluate theWMH pattern of different migraine subtypesMethods: A brain MRI (Siemens, Germany, 3T) was performedin 92 otherwise healthy migraine patients with novascular risk factors (73 females, mean age 34.68.9; 61episodic migraine, 31 chronic migraine, 36 migraine withaura, 56 migraine without aura).Results: The prevalence of WMH in different types ofmigraine was similar and ranged from 38.7% to 44.4%.The distribution of focal WMH decreased from the frontalto the parietal and to the temporal lobe. In most cases,WMH were located in the juxtacortical and/or deep whitematter; only 2 patients had periventricular WMH. WMHappeared as round or slightly elongated foci with a mediansize of 2.5mm [1.5; 3]. Total number, size and prevalenceof WMH by lobes and white matter regions were similarbetween groups, and no interaction with age or sex wasfound.Conclusion: Patients with different subtypes of migraineand without vascular risk factors have a similar pattern ofWMH and no subclinical infarctions and microbleedings,which indicates the low prognostic value of WMH inidentifying a specific migraine subtype or vascular complicationsof migraine. WMH pattern may be used to differentiatemigraine as a primary disorder and other disorderswith migraine-like headache and WMH.
