Аннотация:Представлено клиническое наблюдение пациента, направленного на магнитно-резонансную томографию (МРТ) головного мозга для уточнения причины впервые возникшего эписиндрома. При выполнении МРТ обнаружена эктазия корковых и медуллярных вен левого полушария большого мозга с наличием вокруг последних многочисленных микрокровоизлияний. Выявленные изменения были расценены как проявление регионарной венозной гипертензии, однако ее причина оставалась неясной. Было принято решение выполнить бесконтрастную 3D-TOF-ангиографию, с помощью которой удалось обнаружить дуральную артериовенозную фистулу между левой затылочной артерией и сигмовидным синусом с ретроградным дренажом крови в корковые вены (тип II по Borden, тип IIb по Cognard). Своевременная диагностика и лечение «агрессивных» дуральных артериовенозных фистул позволяют избежать развития жизнеугрожающих осложнений (риск внутричерепных кровоизлияний составляет 8,1 % в год).
